1 & 2: Both are completely random, and it's pretty rare to get the three-form of either.
3: Give it a box to hold, the same boxes from scouring (or the market). The color will match the given box.
4. Yes, but it's a pretty small change. I've given mine a sinister box, you should be able to see it on the back of the belt.
